You have arrived in Japan and found your hotel. You are pretty tired and just want to check in and rest for a few hours. You approach the front desk and are greeted by a young girl. You want to check in. You have a reservation under the name of Bauer.

ともうします is a polite way to state your name. です is said after your state your name for casual social introductions. ともうします is used in business or formal introductions.

ほんじついっぱくのごよていでよろしいでしょうか This question is confirming that you want stay one night from today. よてい generally means "plan". In this case よてい really refers to your intention to stay at the hotel. In short, your reservation/booking. でよろしいでしょうか is the honoriffic form of でいいですか - It it/that OK?
"You're staying with us for one night?"

こちらにごきにゅういただけますでしょうか This question is a request for you to fill in a form. こちら refers to the form that would be handed to you as the request is made. きにゅうする is the verb "to fill in". Adding ご makes the request polite combined with いただけますでしょうか
Requests To make polite requests いただけますでしょうか is often used. It is something you will hear more than say yourself. In any customer service situation or polite social encounter Japanese will use this polite form when making a request.
こちらにごきにゅういただけますでしょうか = Would you mind filling in this?

かかりのものがおへやにごあんないいたします A porter will show you to your room かかりのもの in general means "the person in charge". In this case it would refer to a porter/bell boy. いたします is the polite form of します
